Here's what we do for one Care Oversight client in a typical month.
Week 1: In-person or video functional check-in. Clinical observation of mobility, cognition, home environment, and care quality.
Week 2: Care coordination — physician follow-ups, medication reviews, scheduling, aide check-ins, and any flagged concerns addressed.
Week 3: Family strategy call — clear update on status, what changed, what we're watching, and what decisions may be coming.
Week 4: Written monthly summary delivered. Longitudinal record updated. Available for questions or emerging concerns throughout.
